This role focuses on implementing established care plans, monitoring patient health status, administering medications, and maintaining detailed medical records to ensure optimal patient outcomes and safety. ESSENTIAL FUNCTIONS Provide Direct Care: Monitor patient conditions by measuring vital signs, checking blood pressure, and performing regular physical assessments. Administer Medications: Dispense oral, topical, and intramuscular medications accurately according to physician orders and facility protocols. Manage Wound Care: Clean, treat, and dress wounds, incisions, or lesions utilizing proper sterile techniques to prevent infection. Document Medical Records: Complete accurate, timely charting of patient conditions, treatments administered, and medical histories in the EHR system. Execute Care Plans: Implement specific care plans developed by Registered Nurses (RNs) and physicians while tracking patient progress. Report Status Changes: Observe patients closely and immediately communicate any significant health declines or emergencies to supervising medical staff. Assist Daily Living: Help patients with personal hygiene, bathing, dressing, eating, and safe mobility or transfers. Collect Lab Specimens: Gather blood, urine, or sputum samples following strict infection control and safety guidelines for laboratory testing. Educate Patients: Instruct patients and their families on post-discharge care, medication schedules, and preventative health habits.
