Job Responsibilities • Provide skilled home health physical therapy interventions under the supervision of a licensed Physical Therapist (PT) • Implement and carry out individualized Plans of Care (POC) developed and delegated by the supervising PT • Treat patients with orthopedic, neurological, cardiac, and post-surgical conditions • Administer therapeutic exercises, gait training, balance training, transfer training, and functional mobility activities • Apply manual therapy techniques and non-complex modalities including heat, cold, electrical stimulation, and other physician-ordered treatments • Educate patients and caregivers on home exercise programs (HEP), fall prevention, and safe mobility strategies • Monitor patient response to treatment and promptly report changes in condition to the supervising Physical Therapist • Complete accurate and timely electronic documentation in compliance with Medicare home health regulations • Follow infection control standards and home health safety protocols • Participate in QAPI and agency performance improvement initiatives This PRN Home Health Physical Therapist Assistant job in Dallas \ Fort Worth, TX offers flexibility, field independence, and strong therapist collaboration.
