Fully accredited by The Joint Commission, we offer a full range of services including 24/7/365 Emergency Department and Level IV Trauma Center, hospitalist program, testing, surgery, orthopedics, rehabilitation, and more so that you can experience better health. 

 

Job Summary and Responsibilities

As our Physical Therapist, you will help patients rebuild or maintain their physical wellbeing so they can function at their best level, reduce pain, and improve their quality of life.Every day you will perform initial evaluations, develop and update individualized plans of care, and provide a variety of direct treatments. You will be expected to consistently meet patient care productivity standards, collaborate with the interdisciplinary team and families, and promote a positive customer service environment.To be successful in this role, you must possess strong age-appropriate competency skills, excellent communication abilities, and a collaborative, patient-centered approach to care.

 

Responsibilities:

  • Demonstrates competence with clinical skills necessary to provide care appropriately to all the patient groups served by the physical therapy service
  • Performs evaluations/assessments, develops, re-evaluates and adjusts the Plan of Care for each individual
  • Provides treatments as included in POC
  • Provides instruction in/demonstration of equipment, treatments, disease process, self-care and exercises to patients, family, and staff.
  • Provides timely and accurate documentation of services provided in the medical record, per policy.
  • Communicates with medical, nursing and other Rehab staff

Job Requirements

Required 

  • Graduate of a Physical Therapy program from an accredited institution
  • Physical Therapist licensed to practice in Minnesota
