Compensation Package: Competitive piece-rate pay with multiple bonus opportunities:
- Per Visit: $70 – $130 per completed visit (good-faith estimate; varies by visit type)
- Drive Time: $25 per hour
- Mileage Reimbursement: $0.70 per mile
- Productivity Bonus: Earn additional incentives based on performance
- Documentation Bonus: Reward for timely submission of documentation

Job Summary
As a Physical Therapist (PT) with GrandCare, you will provide in-home physical therapy services according to a physician’s plan of care. You’ll help patients regain mobility and independence after surgery, making a real difference in their recovery.
Responsibilities
- Perform home health physical therapy evaluations and develop care plans
- Provide therapeutic treatment and instruct patients/caregivers
- Monitor progress and report changes to physicians
- Participate in discharge planning and interdisciplinary case conferences
- Supervise Physical Therapy Assistants per state regulations
Requirements
- Current California Physical Therapy License
- 2+ years of physical therapy experience (home health preferred)
- CPR certification
- Valid driver’s license and insured vehicle
