Part-time social worker

Pawtucket Falls Healthcare Center is seeking a compassionate and dedicated Part-Time Social Worker to join our team at our skilled nursing facility. The Social Worker plays a key role in supporting residents and their families by addressing psychosocial needs, assisting with care planning, and helping ensure residents receive the highest quality of care and support during their stay.

Qualifications

  • Bachelor’s Degree in Social Work (BSW) required; MSW preferred.

  • Current Rhode Island Social Work license.

  • 1–2 years of experience in a healthcare or long-term care setting preferred.

  • Strong communication, organization, and interpersonal skills.

  • Ability to work effectively with residents, families, and interdisciplinary teams.

  • Knowledge of long-term care regulations and discharge planning.

  • Experience with electronic medical records (EMR) systems.

  • Strong understanding of resident-centered care.

Responsibilities

  • Conduct psychosocial assessments for residents upon admission and as needed.

  • Participate in interdisciplinary care plan meetings and contribute to resident care planning.

  • Provide emotional support and counseling to residents and their families.

  • Assist residents and families with discharge planning and community resources.

  • Advocate for residents’ rights and ensure their needs are addressed.

  • Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.

  • Collaborate with nursing, therapy, and administrative staff to support resident well-being.

  • Assist with addressing resident and family concerns or grievances.
