Description

Cardiac Rehab: 

Performs patient assessments, including testing, and develops, implements, monitors and evaluates exercise programs for patients. Also provides patient counseling. Participates in the department’s performance improvement and quality assurance / improvement metrics. 

Swing Bed Care: 

Under direct supervision of a Physical Therapist, assist patients with activities of daily living, to promote independence, and highest functional mobility. 

All RRH employees must perform their job responsibilities according to all RRH Hospital policies, as well as to accrediting organizations, federal and state regulation, and to the Centers for Medicare and Medicaid Services (CMS) guidelines, as applicable.


Requirements

Job Qualifications:

Education: 

  • Graduate of an accredited Bachelor Degree program in Physiology, Kinesiology, Nursing, Physical Therapy / Assistant program or related field Technical Training: 
  • Current BLS and ACLS certification, within 6 months of hire
  • Completion of AACVPR Cardiac Rehab module, within 3 months of hire


Prior Work Experience: 

  • One-year experience as cardiac rehab specialist, preferred
  • One-year experience as a Rehab Tech, preferred


Language Skills:

  • Able to communicate effectively in English, both verbally and in writing.
  • Addition languages preferred
  • Excellent interpersonal skills


Skills:

  • Basic computer knowledge


Physical Demands:

  • Duties of the position require extended periods of standing and walking. Repeated bending, stooping and reaching. Pushing and pulling up to 300 pounds and carrying up to 50 pounds. Lifting up to 50 pounds and heavier lifting in transferring or supporting patients who may suddenly become weak or helpless.


Essential Duties and Responsibilities:

Cardiac Rehab: 

  • Performs physical assessments including patient health history.
  • Performs various tests to further assess each patient to develop the appropriate exercise prescription.
  • Develops, implements and evaluates exercise prescriptions for patients under the direction of the Medical Director.
  • Coordinates with the Cardiac Rehabilitation Team and the patient/family to develop an individualized treatment plan for every patient.
  • Identifies and addresses psychosocial needs of the patient and family.
  • Monitors and reassesses patients. Makes revisions in the exercise prescription as needed with the approval of the Medical Director.
  • Prepares home exercise programs with the approval of the Medical Director.
  • Provides patient/family counseling and education classes on cardiovascular risks factors, weight management, medication compliance, smoking cessation classes and others. Motivates and supports patients in the Cardiac Rehab Program.
  • Provides patient and family education related to the exercise program and its benefits
  • Knowledgeable of exercise testing technology.
  • Evaluates new exercises and equipment to enhance exercise programs.
  • Provides in-services for the Cardiac Rehabilitation Team.
  • Coordinates and supervises patient care as necessary.
  • Documentation meets all standards and policies.
  • Maintains records, including statistics; submits reports as requested.
  • Maintains supplies.
  • Follows all other tasks as set by the Director of Physical Therapy


Swing Bed Assist Personnel

  • Assists in performing physical therapy treatments on patients as directed by a physician.
  • Assists with facilitation of basic activities of daily living (ADLs) including bathing, eating, upper garment and lower garment dressing, and toileting.
  • Assists patients in preparing for and during treatments.    
  • Assembles necessary equipment and accessories.     
  • Performs selected treatment procedures as assigned under direct supervision of a Physical Therapist.
  • Assist in other daily tasks of a patient, to promote better health and conditioning
  • Assist with other tasks as set by the Director of Physical Therapy
