Department: Case Management

Schedule: Days | Full-time | Monday to Friday 8 a.m. to 5 p.m.

How you'll make an impact in this role

• Orchestrate continuum of care plans from admission through discharge by aligning clinical pathways with patient and family values while removing barriers to care.

• Evaluate medical necessity and level of care through continuous patient assessments and record reviews to ensure appropriate utilization of healthcare resources.

• Guide seamless post-acute care transitions by serving as the primary liaison between patients, families, and post-acute providers to secure optimal post-discharge services.

• Optimize financial and clinical resources through strategic collaboration with payers, advocating for maximum benefit utilization and efficient, high-value care delivery.

What minimum qualifications you'll need

Licensure / Certification / Registration:

• Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required.
• Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
