What You'll Do As a Home Health Physical Therapist, you'll help patients regain strength, mobility, balance, and independence while making a meaningful impact in their lives. Responsibilities include: Perform comprehensive physical therapy evaluations and reassessments Develop individualized treatment plans based on patient goals and physician orders Provide evidence-based therapeutic interventions to improve mobility, strength, balance, endurance, and function Complete Start of Care assessments and ongoing therapy visits Assess patients' home environments and recommend equipment or safety modifications Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques Collaborate closely with physicians, nurses, occupational therapists, speech therapists, medical social workers, and home health aides Complete timely and accurate electronic documentation Help patients safely recover while reducing hospitalizations and maximizing independence Why You'll Love Working Here At Sunrise, our therapists are supported—not overwhelmed. Qualifications Required Graduate of an accredited Physical Therapy program Current Arizona Physical Therapist license Current CPR Certification Valid Arizona driver's license Reliable transportation and current auto insurance Preferred Home Health experience OASIS experience Strong communication and organizational skills New graduates are encouraged to apply.
