Your Role: Home Health Occupational Therapy Assistant (OTA/COTA)

As a Home Health OTA, you will provide skilled Occupational Therapy services under the supervision of a licensed Occupational Therapist in accordance with physician orders and individualized treatment plans. You will help patients improve functional mobility, safety, strength, and independence within their home environment.

Key Responsibilities

• Provide skilled Home Health Occupational Therapy Assistant services in patients’ homes
• Implement treatment plans established by the supervising Occupational Therapist
• Assist patients with activities of daily living (ADLs), functional mobility, transfers, and upper extremity function
• Reinforce therapeutic exercises, adaptive strategies, and rehabilitation techniques
• Educate patients and caregivers on home safety, fall prevention, adaptive equipment, and energy conservation
• Observe, document, and communicate patient progress and condition changes to the supervising Occupational Therapist and care team
• Complete timely and accurate electronic clinical documentation in compliance with Home Health regulations and company standards
• Collaborate with nurses, therapists, physicians, case managers, and interdisciplinary team members to support patient goals and outcomes
• Maintain professionalism, productivity, and excellent customer service during all patient interactions

Qualifications

• Graduate of an accredited Occupational Therapy Assistant (OTA/COTA) program
• Active and unrestricted Arizona Occupational Therapy Assistant license
• Current CPR certification
• Minimum one (1) year of clinical experience preferred
• Home Health experience preferred
• Strong communication, organization, and time-management skills
• Ability to work independently within a Home Health environment
• Reliable transportation, valid driver’s license, and proof of current automobile insurance
• Ability to travel throughout assigned Arizona coverage areas
