Responsible for assisting in the maintenance of clinical pharmacotherapy programs and services aimed at optimizing therapeutic management for patients at SJUMC and SJWMC. Responsible for delivering direct patient care with a focus on medication therapy management and collaborating closely with other healthcare professionals including nurses and physicians to ensure comprehensive care. Key responsibilities include evaluating patients' medication needs, monitoring progress, and ensuring the safe, effective, and appropriate use of medications across various disease states. Responsible for assisting the Clinical Pharmacy Manager and the Inpatient Acute Care Pharmacy Director in the oversight and execution of all clinical pharmacy activities, in addition to supporting quality improvement and quality assurance initiatives related to the delivery of pharmaceutical care. Responsible for planning, organizing, analyzing, and compiling the facility’s medication utilization programs as necessary, and assisting in the implementation of cost-containment strategies, ensuring continuous monitoring for compliance and performance outcomes.

Responsible for providing clinical information and guidance to physicians, pharmacy personnel, healthcare providers, and patients. Responsible for pharmacy staff education, ensuring the ongoing development of competency within the department. Regularly offers and receives constructive feedback regarding pharmacy practice and patient care issues with the aim of maintaining a high-level practice.

The individual in this role will also participate in various committees and projects as assigned and will be expected to perform other duties as deemed appropriate by the Clinical Pharmacy Manager and the Inpatient Acute Care Pharmacy Director.
