Physical Therapist (PT) | Home Health | Position Details Provides skilled physical therapy services to patients in their homes to restore mobility, improve functional independence, reduce pain, and prevent disability. Evaluates patients, develops individualized treatment plans, and provides evidence-based therapeutic interventions while collaborating with an interdisciplinary care team to deliver compassionate, patient-centered home health services. Key Responsibilities: Evaluate patients to determine physical therapy needs, functional limitations, and rehabilitation goals. Develop, implement, and update individualized plans of care in accordance with physician orders, home health regulations, and physical therapy standards of practice. Provide skilled physical therapy interventions in the home and community setting to improve mobility, strength, balance, endurance, safety, and functional independence. Perform patient evaluations, screenings, reassessments, and treatment documentation in accordance with state, federal, accreditation, and organizational requirements. Delegate appropriate patient care activities to Physical Therapist Assistants (PTAs) and provide ongoing supervision in accordance with Minnesota practice standards. Collaborate with physicians, RN Case Managers, therapy staff, caregivers, and other members of the interdisciplinary team to coordinate high-quality patient care. Educate patients, families, and caregivers on home exercise programs, fall prevention, disease management, equipment use, and strategies to maximize safety and independence. Safely operate, demonstrate, and recommend therapeutic exercise equipment, durable medical equipment, and other assistive devices while ensuring patient and caregiver competency. Participate in interdisciplinary care conferences, department meetings, quality improvement initiatives, and program development activities. Assist department leadership with performance improvement, community outreach, marketing, and promotion of home health therapy services. Provide consultation, education, and training to Perham Living campus staff and other departments as needed. Foster positive patient and family relationships through compassionate, respectful, and professional care while supporting patient satisfaction initiatives. Maintain compliance with all applicable regulatory standards, accreditation requirements, organizational policies, and department standards of behavior. Ensure a safe work environment while providing care in patients' homes. Participate on committees, attend required education and staff meetings, and engage in ongoing professional development. Perform other duties as assigned. What You Offer Graduate of a physical therapy education program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE) or its successor organization, or an equivalent program recognized under Minnesota licensure requirements. Current licensure or eligibility for licensure as a Physical Therapist in the State of Minnesota. Minimum of one (1) year of clinical experience preferred; home health or geriatric experience is strongly preferred. Demonstrated ability to evaluate and treat a broad range of patient populations, including adult and geriatric patients. Strong clinical assessment, documentation, communication, and organizational skills. Ability to work independently while collaborating effectively within an interdisciplinary healthcare team. Ability to safely operate and instruct patients in the use of therapeutic exercise equipment, durable medical equipment, and assistive devices. Willingness to participate in community education, marketing activities, and professional development opportunities. Valid Minnesota driver's license, reliable transportation, proof of current automobile insurance, and the ability to travel in all weather conditions. Maintain professionalism, confidentiality, and excellent customer service. Communicate effectively in English, both verbally and in writing. Pass a background study through the Minnesota Department of Human Services.
