Forestdale (FD) is a NYC child welfare agency working to prevent child abuse/neglect through parenting, prevention, and foster care (FC) services, focusing on low-income and immigrant communities in Queens & Brooklyn. We help thousands of families work through acute & complex trauma, parents raise the next amazing generation of children, and youth successfully launch into adulthood. Job Summary

Case Planners will receive specific training in evidence-based and evidenced-informed interventions that will assist the Case Planner support families and caregivers reach permanency goals within a reasonable period of time. S/he will be responsible for planning for families with the intent of achieving a permanency for children either through reunification with birth parents/discharge resources, and if not possible, through adoption, KinGap or APPLA. Ā 

Salary Range: Ā  Ā 

Bachelors - $29.12/hr - $31.04/hr

Bachelors (Bilingual)- $30.22/hr - $32.15/hr

Masters - $30.22/hr - $33.24/hr

Masters (Bilingual) - $31.59/hr - $34.34/hr

Masters (Bilingual & License) - $34.43/hr - $37.09/hr

Primary Responsibilities

• Ā  Ā  Ā Conduct field visits to foster homes and birth parent's homes

• Ā  Ā  Ā Coordinate and supervise family visits at the agency and/or in the community

• Ā  Ā  Ā Facilitate weekly groups for birth parents and foster parents

• Ā  Ā  Ā Ensure each case adheres to ACS’ permanency timeline ASFA guidelines

• Ā  Ā  Ā Maintain case compliance via data entry and record keeping Ā 

• Ā  Ā  Ā Accompany families to necessary drug treatment, medical, school, housing, public assistance

Ā  Ā  Ā  Ā service appointments as necessaryĀ 

• Ā  Ā  Ā Attend internal trainings as scheduled

• Ā  Ā  Ā Attend court hearings with prepared court reports and/or permanency reports; proficiently reportĀ 

Ā  Ā  Ā  Ā and/or testify to the Court the progress of each case and any noted barriers to accomplishing the Ā Ā 

Ā  Ā  Ā  Ā goals of the service plan

• Ā  Ā  Ā Participate in family team conferences, transition conferences and goal change conferences

• Ā  Ā  Ā Present at weekly case conferences

• Ā  Ā  Ā Complete other duties as assigned

Minimum RequirementsĀ 

• Ā  Ā  Ā Bachelor's or Master's degree in social work, psychology, criminal justice or counseling from anĀ 

Ā  Ā  Ā  Ā accredited college; related field

• Ā  Ā  Ā Ability and motivation to facilitate birth parent and/or foster parent support groups, utilize familyĀ 

Ā  Ā  Ā  Ā finding techniques and successfully manage cases to comply with permanency planningĀ timelines.

• Ā  Ā  Ā An understanding and respect for community needs and cultures

• Ā  Ā  Ā Organizational, verbal and written communication skills

• Ā  Ā  Ā Ability to accommodate a flexible work schedule, including evenings and weekendsĀ 

• Ā  Ā  Ā Ability to travel in and around the metropolitan New York City region

• Ā  Ā  Ā Ability to maintain composure and function well under stress

• Ā  Ā  Ā Fluency in Spanish, Mandarin, Cantonese or Russian a plus
