POSITION SUMMARY:

 

The Chronic Care Case Management Nurse supports Medicare patients through assessment, planning, facilitation, and advocacy to promote effective, cost‑efficient health outcomes. This role helps patients understand and use their health plan benefits; coordinates needed services and works to remove barriers to appropriate care. The nurse functions both independently and collaboratively within a multi‑specialty clinic environment and may rotate among clinics, primarily conducting non–face‑to‑face encounters with the Medicare population.

 

 

EDUCATION & EXPERIENCE:

 

  • Graduation certificate from an accredited Registered Nurse (RN) program, required
  • Current MS state licensure, required
  • CPR, ACLS, and IV certifications, preferred
  • Three (3) years of prior experience, preferred

  • Strong multitasking and independent‑work capabilities
  • Excellent written and verbal communication skills
  • Accurate documentation skills
  • Ability to follow established workflows consistently
  • Knowledge of EMR, medical management and policies and procedures
  • Intermediate skills in work processing, spreadsheet and database development and maintenance

 

WORK SCHEUDLE:

 

Monday - Friday, 8:00AM - 5:00PM.
