We are seeking a compassionate and dependable Physical Therapist Assistant to join a Skilled Nursing Facility in Petaluma, California This position offers the opportunity to work with a diverse geriatric and post-acute population, assisting patients in recovering from surgery, illness, or injury. Under the supervision of a licensed Physical Therapist, you will provide hands-on care focused on improving mobility, strength, and overall functional independence. Job Responsibilities:
• Assist the Physical Therapist in implementing individualized treatment plans in a Skilled Nursing Facility setting
• Provide therapeutic exercises, gait training, and functional mobility activities
• Monitor patient progress and communicate any changes or concerns to the supervising Physical Therapist
• Educate patients and caregivers on exercises, safety, and proper mobility techniques
• Maintain accurate and timely documentation of treatments and patient progress
• Ensure compliance with facility policies and regulatory requirements




Requirements

• Active state license as a Physical Therapist Assistant in California
• Current BLS/CPR certification
• Minimum of 6 months of recent PTA experience within the last 5 years preferred
• Experience in a Skilled Nursing Facility or geriatric setting is a plus
• Strong communication and teamwork skills
