Founded in 1970 as South Philadelphia Health Action and subsequently incorporated as Greater Philadelphia Health Action, GPHA is a non-profit healthcare organization committed to providing compassionate and comprehensive healthcare services regardless of an individual’s ability to pay.  Since 1970, GPHA has expanded to become one of the premier providers of primary care, specialty care, dental services, and behavioral healthcare in the Greater Philadelphia area. 

We are presently seeking full-time Community Health Navigators. 

Community Health Navigators enhance the quality and utilization of HIV services by providing counseling, outreach, and referral services for patients and their support systems.

Must have a high school diploma, preferably with at least one to two years’ experience working with HIV or OB patients. Experience in integrated health services, social determinants of health assessment, outreach, caseload management and/or tracking is a plus. Candidates should be able to travel to multiple sites throughout GPHA's healthcare network and work proactively both independently and as part of a team.
