The AOT Outreach Nurse Coordinator works closely with medical and program leadership. Supporting the liver and kidney transplant programs by coordinating the delivery of care for the transplant patients in the satellite clinics throughout the Temple service area. Serves as a liaison between Temple University Hospital and our partner organizations to improve the delivery of care. Under the direction of the Director Clinical Transplant Services, partners with the QAPI team to develop and report on quality improvement projects related to Temple's AOT satellite clinics. Supports the outreach and education coordinator as needed to provide clinical support for dialysis center visits. Demonstrates a thorough knowledge of nursing theory and practice involved in providing nursing care. Familiarity with organization and function of departments, policies, procedures and regulations. Orchestrates the care of patients with end stage organ disease. This position requires regular travel between Temple University Hospital and Satellite locations.

Education
Bachelor's Degree in Nursing from an accredited Nursing program. Required
Master's Degree in Nursing from an accredited Nursing program. Preferred

Experience
3 years experience in nursing. Required
General Experience and knowledge of Electronic Medical Record. Required
General Experience in a Transplant program. Preferred

Licenses
PA Registered Nurse License Required
Multi State Compact RN License Required Or
