Woodstock Family Center (WFC) provides emergency housing and supportive services to families experiencing homelessness in Philadelphia. During their stay, residents focus on achieving permanent housing, increasing employment opportunities, improving health and wellness, strengthening family stability, and connecting with community resources.

Through a person-centered and trauma-informed approach, Woodstock Family Center helps families create sustainable plans for long-term success and independence.

What You'll Do

As a Case Manager, you will:

  • Serve as part of a multidisciplinary team providing holistic, person-centered services.
  • Assist residents in developing individualized recovery, housing, and service plans.
  • Support families in identifying goals and overcoming barriers to stability.
  • Connect residents to housing resources, employment supports, healthcare providers, educational services, and community programs.
  • Maintain relationships with community agencies and referral partners.
  • Advocate for residents to ensure access to necessary services and supports.
  • Provide community-based case management and resource coordination.
  • Monitor progress toward service plan goals and assist with plan updates.
  • Complete assessments, case notes, reports, and required documentation in accordance with organizational, licensing, and funding requirements.
  • Participate in identifying service gaps and recommending strategies to improve outcomes.
  • Ensure access to intensive case management services as required by program standards.
  • Perform additional duties as assigned.

Qualifications

Required

  • Bachelor's Degree in Human Services or a related field.
  • Minimum of two years of experience providing support to individuals and families in behavioral health, addiction recovery, housing, homeless services, intellectual and developmental disabilities, or related human services programs.
  • Valid Driver's License and acceptable driving record.
  • Intermediate computer skills.
  • Strong verbal and written communication skills.
  • Ability to work effectively in community-based environments.
  • Ability to work collaboratively while maintaining independent case management responsibilities.

Preferred

  • One year of Case Management experience.
  • Experience working with individuals and families experiencing homelessness.
  • Knowledge of community housing resources and social services.
  • Experience within behavioral health, recovery, or family-focused programs.

Schedule

  • Full-Time
  • Community-Based Position
  • Flexible schedule based on resident and program needs
  • Participation in after-hours service availability as required by program expectations

Physical Requirements

  • Ability to sit, stand, walk, bend, reach, and climb stairs.
  • Ability to drive throughout the service delivery area.
  • Ability to lift up to 50 pounds occasionally.
  • Ability to transport residents to appointments and community resources as needed.
  • Ability to perform essential job functions with or without reasonable accommodation.
