JOB SUMMARY

The Community Wellness Coordinator supports the ECS Bridge to Prosperity® model and Health & Well-Being pillar by coordinating wellness programs, recruiting and engaging participants for program enrollment, connecting participants and community members to ECS services and community-based resources, and supporting community engagement activities. This role manages participant referrals, program scheduling, data collection and reporting, partner and facilitator communication, and outreach efforts that promote health, well-being, and family stability.

The Coordinator provides administrative and operational support for community wellness initiatives, including community dinners, parenting programs, nutrition education, and other health and wellness activities. The role also serves as a primary liaison for key partnerships, including the Parent Collaborative with PHMC, by supporting communication, participant engagement, scheduling, reporting, and program implementation.

Essential Job Duties and Responsibilities

  • Support the ECS Bridge to Prosperity model (adapted from EMPath’s Mobility Mentoring Model) throughout ECS by modeling a coaching culture for peer staff and as a part of participant programs and services.
  • Coordinate and support group Health and Wellness workshops to help participants across all ECS programs enhance their health literacy regarding parenting, diabetes, nutrition, and other health and wellness topics.
  • Develop curricula and identify training materials for health & wellness programming.
  • Connect participants and community members to Health and Wellness services to advance their health and family goals.
  • Enter data promptly and accurately to enable continuous data collection, tracking, reporting, and compliance monitoring for department goals and initiatives.
  • Maintain relationships with program and community partners in the Greater Philadelphia region to provide health education, resources, and support for ECS participants and community members.
  • Monitor data and alignment of results to expected outcomes to inform program modifications and continuous quality improvement efforts.
  • Coordinate community wellness programming and engagement activities, including community dinners, health and wellness initiatives, resource fairs, and other participant and community-focused events that support ECS program goals and intended outcomes.
  • Collaborate with Marketing & Communications, Neighborhood Advisory Committee (NAC) staff, Housing, Opportunity Development, consultants, volunteers, and community partners to plan, promote, implement, and evaluate health and wellness programs and community engagement activities.
  • Perform community outreach and engagement activities to recruit participants for ECS programs, support program enrollment, increase awareness of ECS services, and connect community members to health and wellness resources.

Education and Experience

  • A high school equivalency or diploma is required for this position; a Bachelor's Degree in the related field is preferred.
  • A minimum of two (2) years of experience in health promotion, social or human services, or a related work area.
  • Minimum of one (1) year of experience with coaching or providing health & wellness services.

Skills and Abilities

  • Experience in and knowledge of the local healthcare landscape and health equity is preferred.
  • Knowledge or awareness of local and national organizations that support or promote healthy lifestyles and address health equity and the social determinants of health is preferred. 
  • Exposure to program logic models, program evaluation, and quality improvement.
  • Excellent written and verbal communication skills.
  • Strong interpersonal skills with the ability to build and maintain relationships.
  • Ability to communicate and work effectively in a variety of settings with diverse staff,
    Participants, visitors, and communities.
  • Knowledge of the effects of intergenerational poverty on individuals and families in
    Philadelphia.
  • Excellent self-direction, follow-through, and ability to take ownership of given responsibilities. 
  • Strong organizational skills with the ability to manage multiple projects simultaneously.
  • Ability to prioritize projects and tasks and to respond to simultaneous requests from several directions.
  • Advanced computer skills, especially in Windows, MS Office (Word, Excel, PowerPoint, Access), MS Teams, and Zoom platforms.
  • Ability to represent ECS effectively in various settings and with diverse communities.

Travel and Schedule

This role may require travel to other ECS locations for training, agency-wide events, etc. Travel to various locations throughout Philadelphia is required to attend partnership events and activities. 

Monday - Friday with limited evenings and weekends as needed for programming events.
