JOB SUMMARY:

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member to provide quality services. Assist in the coordination of all intake services for OP. Provide inpatient liaison services when individuals receiving OP services do not have other types of CM. Coordinate linkages to other agencies, levels of care, and resources as needed. In collaboration with COC Specialist II and OP staff, provide crisis management for individuals receiving OP services. Maintain the Specialized Case Management caseload, maintaining communication with outside treatment team and family members for continuity of care & ensuring all documentation remains in compliance.

QUALIFICATIONS:

Bachelor’s degree in human Services or related field required; 

Minimum of two years experience working in human services with some quality improvement preferred; 

Ability to use Electronic health Record required.

Basic Word, Excel and Power Point Skills desired; Knowledge of program evaluation, continuous quality improvement, and managed care procedures related to the behavioral health field preferred.  Excellent writing and analytical skills required.  Attention to detail.

Driver's License Required; Vehicle Preferred but not required.  

PHYSICAL DEMANDS:  

Minimal in nature.

SPECIFIC DUTIES:

1. Provide linkages to other agencies and services as needed

2. Utilize the ASAM assessment tool with individuals who need substance use services, as credentialed

3. Provide specialized case management to individuals who have developmental disabilities and receive services through CLAs at non-PATH organizations, ensuring coordination of care and completion of all required documentation for care.

4. Coordinate crisis, inpatient liaison, and case management services with other staff in COC/OP Department

5. Participate in crisis assessment to ensure individual receives necessary supportive intervention and assistance

in managing and resolving the crisis and develop a crisis plan to assure coordination of recommended

disposition

6. Function as a member of the agency’s internal crisis team and after successful completion of mandatory

training restrain, or place in restraints, those individuals who are unable to control themselves and present

danger to themselves or others.

7. Communicate with your supervisor about individuals needing higher level review

8.. Collaborate with financial liability department regarding insurance and billing issues

9. Schedule intake assessments by screening individuals over the phone or through the walk-in clinic

10. Provide coverage of intake/crisis phones

11. Assist individuals with gaining access to needed psychiatric, medical, social, residential, financial and other

services as appropriate for the individual

12.Ensure collaboration with individuals’ treatment teams, including the psychiatrist when coordinating care

13. Notify supervisory staff of emergencies and urgent issues in a timely manner

14. Complete or assist others in completing an involuntary commitment petition

15. Make approved home visits

16. Maintain and update resource file

17. Other Duties as assigned
