The FaSST/Connections Program provides Intensive Targeted Case Management services to families and individuals throughout Philadelphia who require mental health support services. Our team works closely with consumers, natural support systems, and community providers to ensure access to the services needed for long-term stability and recovery.

What You'll Do

As an Intensive Case Manager, you will:

• Assess and evaluate the needs of individuals assigned to your caseload
• Assist consumers in developing and achieving personal goals
• Link individuals to psychiatric, medical, social, residential, financial, and community-based services
• Monitor services and supports to ensure effectiveness and appropriateness
• Complete all required state-mandated documentation and service records
• Conduct outreach activities, including home visits and community-based support
• Help consumers establish and strengthen natural support networks
• Advocate on behalf of consumers to obtain critical resources and services
• Collaborate with family members, support systems, and service providers
• Monitor consumer progress and identify changes that may impact treatment or recovery
• Participate in crisis prevention and de-escalation efforts as needed
• Perform other related duties as assigned

Qualifications

Required

• Bachelor's Degree in a Human Services field
• Valid Driver's License
• Knowledge of homelessness, addiction services, residential programs, mental illness, and treatment resources
• Experience with crisis prevention and de-escalation techniques

Preferred

• Previous case management experience
• Experience working with individuals experiencing severe mental health challenges
• Familiarity with community-based behavioral health services

Schedule

• Full-Time position
• Community-based role with field work and home visits
• Availability to support consumers throughout the community
• Participation in on-call or after-hours responsibilities as required by program needs

Physical Requirements

• Ability to travel throughout the service area
• Ability to conduct community visits and home visits
• Ability to sit, stand, walk, bend, and carry work-related materials as needed
• Ability to perform essential job functions with or without reasonable accommodation
