The Obstetrics Nurse Navigator provides individualized clinical navigation, patient education, and care coordination for obstetric patients across the ambulatory care continuum. This role serves as a central point of contact for patients, families, providers, nursing, scheduling, authorization, and ancillary teams to support timely access to care, follow-up, and clear communication. The Nurse Navigator identifies and helps address barriers to care, coordinates referrals, diagnostic testing, procedures, and follow-up needs, and supports patients through pregnancy, postpartum, and other obstetric care pathways. The role contributes to quality, patient experience, care gap closure, and operational efficiency by promoting proactive outreach, organized follow-up, and coordinated interdisciplinary care.-Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson
-Provides competent clinical care coordination and nursing support to obstetric patients during office visits, telephone encounters, and electronic communications.
- Serves as a patient educator and clinical resource for patients and families regarding diagnoses, treatment plans, procedures, referrals, testing, follow-up care, and next steps.
- Coordinates timely access to appointments, diagnostic testing, imaging, procedures, referrals, authorizations, and specialty consultations in collaboration with providers, scheduling, and ancillary departments.
- Supports patients in navigating the healthcare system by identifying barriers to care, escalating concerns appropriately, and connecting patients with available resources.
- Conducts outreach and follow-up for patients with missed appointments, abnormal results, emergency department visits, hospitalizations, postpartum needs, high-risk clinical findings, or other care gaps identified by the clinical team.
- Ensures necessary clinical records, imaging, pathology, laboratory results, referral documentation, and other materials are available for provider review and patient visits as appropriate.
- Relays patient messages, clinical concerns, and care coordination needs to clinicians and follows up to ensure timely communication and resolution.
- Collaborates with physicians, advanced practice providers, nursing staff, medical assistants, patient access, financial clearance, social work, and other interdisciplinary partners to support continuity of care.
- Tracks obstetric patient populations and quality-related follow-up needs, including postpartum visit completion, high-risk patient monitoring, prenatal care gaps, and other identified departmental metrics.
- Supports outpatient symptom management, transitions of care, prenatal and postpartum follow-up, and proactive communication to help reduce avoidable emergency department utilization, delays in care, and unnecessary readmissions.
- Supports program growth, patient experience, and process improvement efforts by identifying workflow barriers and participating in initiatives that improve access, communication, and care coordination.
Job Description
Education:
Bachelor’s Degree in Nursing required.
AND
Experience:
Minimum of 3 years of nursing experience in ambulatory care, obstetrics, maternal health, care coordination, patient navigation, or a related clinical setting required. Prior nurse navigation, case management, population health, prenatal care coordination, or postpartum follow-up experience preferred.
Minimum Certifications, Registration or License Requirements:
Registered Nurse in Pennsylvania required.
