Job Summary
Performs Home visits for trauma patients post-discharge. Participates in community education on injury prevention and trauma recovery. Coordinates with local health agencies, shelters, and social services. Performs mental health screening and referrals. Provides support for victims of violence and domestic abuse. Provides follow-up care for trauma patients. Wound care, medication management, and rehabilitation coordination. Participates in multidisciplinary team collaboration, Trauma Clinic, and Trauma Quality of Life Clinic. Ensures documentation and data collection for quality improvement.
Education
路聽Bachelors Degree : in Nursing from an accredited school of nursing. (Required)
路聽Masters Degree : in Nursing from an accredited Nursing program. (Required)
Licenses and Certifications
路聽CRNP - PA Cert Reg Nurse Practitioner (Required)
路聽RN-LIC - PA Registered Nurse License (Required)
路聽PRX-AUTH - Prescriptive Authority (Preferred)
路聽BLS - Basic Life Support (Required)
Experience
路聽2 Years experience as a nurse practitioner (Preferred)
路聽1 Year experience in a clinical specialty practice (Preferred)
路聽1 year outpatient/ambulatory experience (Preferred)
路聽Experience in an EPIC environment (Preferred)
