SUMMARY

The Infusion Pharmacist is responsible for providing safe and effective pharmacy services in accordance with company policies and procedures, state and federal regulations, pharmacy practice standards, and accrediting entities. The Infusion Pharmacist coordinates pharmacy activities including, but not limited to, patient onboarding, sterile compounding, packing/shipping, and ongoing patient clinical monitoring.

SCHEDULE
Monday - Friday, 8:30 AM - 5:00 PM

ESSENTIAL FUNCTIONS

  • Provides direct oversight of Pharmacy Technicians and support staff.
  • Assesses patients for suitability for home care.
  • Designs patient-specific care plans and drug monitoring regimens. Recommends changes to therapy or monitoring plans as appropriate. Modifies care plans as needed to ensure progress toward therapy goals.
  • Communicates with Pharmacists, Pharmacy Technicians, referral sources, patients, caregivers, and other relevant entities to provide safe, high-quality care.
  • Coordinates with nursing staff to ensure the most cost-effective delivery methods are used to achieve expected outcomes.
  • Oversees the drug preparation work of Pharmacy Technicians and verifies accuracy before medication release.
  • Prepares medications that are appropriately concentrated, stable, free from contaminants, and stored correctly. Labels final products, including ancillary labels and storage instructions. Inspects the final product before dispensing.
  • Ensures compliance with FDA, DEA, State Board of Pharmacy, OSHA, and other regulatory standards. Follows department policies and participates in staff meetings and required functions. Maintains licensure, continuing education, and required competencies.
  • Complies with all federal and state laws regarding controlled substance dispensing.
  • Provides accurate and timely responses to drug information inquiries from patients and providers.
  • Ensures the accuracy of pharmacy support personnel’s work.
  • Ensures compliance with manufacturer requirements for REMS or other requirements when dispensing or shipping drugs.
  • Oversees the safety, appropriateness, and effectiveness of pharmaceutical care related to specialty products dispensed.
  • Participates in the on-call rotation.
  • Performs other duties as assigned.

EDUCATION AND EXPERIENCE

Basic Requirements:

  • Pharm.D. or BS Pharmacy degree from an accredited college of pharmacy.
  • Successful completion of Pennsylvania state pharmacy board examination.
  • Pharmacist license current and in good standing in the state of Pennsylvania.
  • One year of experience in a clinical pharmacy setting.

Preferred Qualifications:

  • Completion of an ASHP-accredited residency program.
  • Experience in a hospital, home infusion, or sterile compounding environment.

COMPETENCIES

  • Knowledge of federal, state, and local laws and regulations applicable to pharmacy practice.
  • Proficiency in dispensing computer software.
  • Proficiency in all areas of pharmacy compounding.
  • Thorough understanding of controlled substance rules and regulations (DEA, state, and local).
  • Expertise in pharmacotherapeutics to oversee the pharmacy patient management program.
  • Leadership skills.
  • Strong time management and organizational abilities.
  • Professionalism.
  • Effective oral and written communication skills.
  • Ability to prioritize and delegate tasks effectively.
  • Friendly and caring demeanor.
