Greater Philadelphia Health Action, Inc. (GPHA) is presently seeking:
- A full-time Registered DietitianÂ
- Collaborative member of a multidisciplinary healthcare team comprised of primary care clinicians (Pediatric, Internal/Adult Medicine, Family Medicine) and specialists (Clinical Pharmacy/Endocrinology – Infectious Disease – Rheumatology – Podiatry). The healthcare team also includes medical case managers, nurses, and behavioral health therapists.Â
- Nutrition evaluation and counseling, without limit, to patients of all ages, including but not limited to:
- Diabetes, prediabetes
- Hypertension, hyperlipidemia
- Prenatal and postpartum nutrition
- ObesityÂ
- Abnormal weight loss/failure to thrive
- Nutritional supplementationÂ
- Nutritional impact of social determinants of health, medication and behavioral health disorders
