The RN Case Manager: provides quality case management and qualified professional support and services to inpatient social workers as assigned; works with local agency and service providers to facilitate sending and receiving collateral information, treatment referrals, and ongoing communications to support patient’s successful inpatient treatment and discharge.

 

  • Review clinical documentation from referral sources to ensure direct admissions are appropriate for level of care
  • Coordinate bed availability and placement with unit nurses and nursing leadership.
  • Meet with patients to provide education on initial treatment planning and safety goals.
  • Prioritize bed placement based on patient acuity, safety considerations, and census needs
  • Participate in quality improvement initiatives by reviewing nursing assessment documentation, patient consents, and ensuring insurance pre-certifications are completed.
  • Provide a thorough report of patient presentation and necessary safety precautions to unit nurse to support a safe transition to the unit.

Qualifications

  • Registered Nurse degree and license
  • A graduate of a college, or university with a Masters degree in a human services field and has one year of full-time, post graduate accumulated mental health experience with the population served and a substance abuse professional shall have one year of full-time post-graduate accumulated supervised experience in alcoholism and drug abuse counseling;
