Assists in meeting the psychosocial needs of patients/families and enables patients to utilize health care in order to achieve their optimal level of health.

• Master's degree from an accredited school of social work.
• Current PA license required
• 1-2 years Post graduate degree, experience n a health care setting, (hospital/home care preferred).
• Flexible, sound judgment, customer oriented, organized, keeps professional demeanor at all times.
• Excellent verbal and written communication skills.
• Available to work flexible hours, with weekends and holidays included.
• Knowledge of general disease process.
• Knowledge of family dynamics and interpersonal relationships.
• Member of the Academy of Certified Social Workers, signifying two years of practice under qualified supervision preferred.
• Able to travel to patient's home within service area.
• Able to use office equipment; i.e. beeper, car phone.
• Valid driver's license.
