Requirements MUST be enrolled in PCF Please note that every bid MUST have the following information listed on each bid.-Phone number-Active License Number-Active state DEA Number-Active CSR -The board verification-Email-Address (provider must be 90+ miles from facility)-DO/MD-DOBQualifications/Requirements for Mammography Radiologists•Must have no less than 960 mammograms dictated in the last 24 months•Must have 15 current/valid mammography specific CME's in last 36 months•Proof of eight hours of digital mammography training•Proof of eight hours of tomosynthesis mammography training•Current state license•American board certification•Residency letter or training attestation which contains proof of:othree months of training in the interpretation of mammography including instruction in radiation physics, radiation effects, and radiation protection to include 60 hours of education in mammography and 240 mammo dictated completed within the last two years of residency.If your bid is accepted, you must submit the credentialing documents by the deadline given by the onboarding coordinator. All credentialing documents are inthe "other documents" tab. ***You can start this process before your bidis accepted***Provider must be contracted with the agency prior to placing a bid. ***PLEASE NOTE: There will be no OT rate for this assignment request***If your bid is accepted, you must submit the credentialing documents 7 daysafter acceptance as this is a new start for CHS HBP. Your bid runs the risk ofcancelation if all onboarding documents are not uploaded within the 7-daytimeframe.
