Job Summary and Responsibilities

As our Care Coordination Licensed Clinical Social Worker (LCSW) at St. Joseph's Hospital & Medical Center, you will be a vital advocate for patient well-being. You will serve as a key member of our interdisciplinary healthcare team, utilizing your clinical expertise to provide high-quality social work interventions, comprehensive psychosocial assessments, and strategic transition planning for inpatients, outpatients, and emergency department patients. This role is essential to our mission of delivering compassionate, developmentally appropriate care to vulnerable populations while driving successful health outcomes across the care continuum.

 

Every day you will manage a diverse caseload, performing critical tasks such as crisis intervention, high-risk screening, and brief counseling to support patient mental health and recovery. You will actively facilitate complex discharge planning, coordinate community resources to address social determinants of health—such as housing and food insecurity—and lead family care conferences to ensure seamless continuity of care. By providing expert education on health management and navigating insurance complexities, you will play a proactive role in readmission prevention and optimizing the overall patient experience in a fast-paced clinical environment.

Job Requirements

Required

  • Two (2) years Post-MSW experience 
  • Licensed Clinical Social Worker (LCSW): AZ
  • Master's Degree from a school of social work accredited by the Council of Social Work Education
  • Ability to work independently as needed with a high degree of detail orientation
  • Ability to work efficiently in a fast-paced environment with changing priorities 
  • Ability to work collaboratively with an interdisciplinary care team

Preferred

  • Minimum of five (5) years Post-MSW healthcare experience
