Job DetailsJob Location: Downtown Medical Respite Center - Phoenix, AZ 85007Summary of Position: Under the direction of the RN Program Manager, the HUC's primary responsibilities are to act as a liaison between the patients and staff. They must have excellent communication skills to serve as this vital link between the physicians, nursing staff, patients, and community partners. Assumes responsibility and accountability for incorporating the organization's vision, values, mission, critical goals, and behavioral standards into their job performance. Essential Duties: Duties include, but are not limited to: Manage and maintain patient records in an organized and confidential manner. Create in-house appointments for all patients upon admission. Handle and direct phone calls and other communications in a timely and professional way. Receive and distribute messages and results from laboratory and radiology departments. Liaise between healthcare professionals and other team members within the unit to facilitate smooth communication and coordination of care. Create and follow up on referrals generated from providers to ensure patients appointments are received. Process and complete any needed prior authorizations via insurance companies. Schedule and coordinate appointments and procedures, ensuring that patients are informed and prepared. Manage transportation calendar and create schedule for the Medical Van Driver. Communicate any update in a timely manner. Aid in coordination of discharge paperwork in conjunction with the case managers and clinic staff. Update EMR upon patients discharge. Complete and distribute all daily and monthly reports (box report, census, driving schedule, vitals log, etc.). Maintain and organize supplies and equipment. Ensure that the unit complies with all relevant regulations and guidelines. Provide general administrative support, such as filing, data entry, and record-keeping. QualificationsQualifications: Basic Knowledge and Skills: Prior experience as a Health Unit Coordinator or related administrative role in a healthcare setting preferred. Familiarity with basic medical terminology and knowledge of HIPAA regulations. Exceptional communication skills and strong attention to detail. Strong organizational and problem-solving skills, with the ability to multi-task and prioritize responsibilities in a fast-paced environment. Comfortable with the use of electronic health records and other healthcare-related software, as well as office productivity suites such as Microsoft Office. Ability to work as part of a team and promote a positive and professional work environment. High school diploma or equivalency. Bilingual (Spanish/English) preferred. Must be at least 21 years of age. Physical and Mental Requirements: Position requires extended periods of sitting and standing. Position requires regular bending and reaching.
