Job Summary
We are seeking an Occupational Therapist to join our home health care team! As an Occupational Therapist, you will meet with patients each day at their home, discuss their treatment needs and ultimate goals, and craft customized, comprehensive therapy plans for them per the doctor's order. You will also work closely with other home health care members of their health team to arrange follow-up care, help educate patients on lifestyle changes they may need, and provide a positive, affirming presence in their lives. The ideal candidate has a compassionate approach to care, strong communication and interpersonal skills, and is comfortable handling multiple cases at once. 

Responsibilities 
  • Meet with patients from many different backgrounds and compassionately discuss their health concerns and therapy needs
  • Conduct evaluations to determine a starting point for each patient
  • Create a therapy plan specific to each patient that will help them meet their mobility goals
  • Work closely with other members (RN, OT, ST) of each patient’s care team to provide adequate follow-up care
  • Maintain excellent records of patient therapy and progress
Qualifications
  • Must hold the required state license for occupational therapy
  • Strong communication and interpersonal skills
  • Excellent organizational skills
  • Good attention to detail
  • The ability to stay positive and compassionate over a long period of time with patients
