About The Position

This opportunity begins on May 12, 2025, and spans 90 days with potential for extension. The position involves two providers working 10-hour shifts, with one covering the first three days of the week and the other covering the last four days. The ideal candidates will be comfortable conducting telehealth visits, managing chronic conditions, and providing remote patient care.

Job Details: 

Location: Arizona (Telehealth services) 

Facility Type: Urgent Care 

Start Date: May 12, 2025

LOA: 90 days 

Provider Type Needed: Family Practice Nurse Practitioner (2 providers) 

Schedule: 10 hours per day, 7 days a week (split between 2 providers)

Responsibilities and Duties: 

• Conduct telehealth visits via Facetime or similar platforms

• Assess patient needs, symptoms, and overall health status remotely

• Prescribe medications and adjust treatment plans as necessary

• Provide consultations and recommend oxygen therapy when appropriate 

• Manage chronic conditions through Chronic Care Management (CCM) protocols 

• Perform 10-15 minute telehealth visits efficiently and effectively 

• Offer home care guidance and support to patients 

• Maintain accurate and detailed medical records 

• Collaborate with on-site staff and other healthcare professionals as needed 

• Adapt to the unique challenges of remote patient care

Additional Information:

• Licenses required in Arizona, Florida, New Jersey, or New York

• Strong experience in telehealth and urgent care is essential 

• Comfort with technology and virtual patient interactions is crucial

• Flexibility to work either the first three or last four days of the week
