Greenfield Rehabilitation Agency is looking for a passionate PRN Physical Therapist to join our team supporting senior communities in Pigeon Falls and surrounding areas. As a PRN PT, you’ll provide skilled, patient-centered care across skilled nursing facilities, assisted living communities, and in-home care, helping residents improve mobility, strength, and independence while enhancing quality of life.

Your Role:

• Conduct initial and ongoing evaluations to assess mobility, balance, strength, and function

• Develop and implement individualized treatment plans

• Provide hands-on therapy to help residents recover, maintain independence, and prevent falls

• Educate residents, families, and staff on safe mobility and functional activities

• Collaborate with PTs, OT, SLPs, nurses, and other staff to ensure coordinated, holistic care

• Maintain accurate documentation of therapy sessions and outcomes

Requirements:

• Licensed or eligible for licensure as a Physical Therapist in Wisconsin

• Strong clinical skills with a passion for helping older adults

• Excellent communication, collaboration, and problem-solving abilities

Qualifications

• Graduate of an accredited school with a bachelor’s, master’s, or doctoral degree in Physical Therapy.
• License as required by the State of Wisconsin to practice as a Physical Therapist.
