We provide residential and community-based services to individuals with intellectual disabilities and mental health challenges. We are committed to enhancing the quality of life for those we serve by offering a range of treatment and support services. Job Summary Strawberry Fields, Inc. is seeking a Blended Case Manager (BCM) with experience working with children and adolescents to support individuals with serious and persistent mental illness. This position will involve working with a caseload that is 60% children/adolescent and 40% adult consumers. As a BCM, you will assess consumer needs, create tailored service plans, and provide ongoing support to ensure those needs are effectively met. You will work closely with individuals and families, coordinating resources and services to ensure optimal outcomes. Vehicle Required: A privately maintained vehicle is necessary for daily client visits within the Centre County area. Candidates will possess a valid PA driver’s license with 3 years of driving experience and excellent driving history.
