This role is ideal for licensed practical nurses who prefer weekend shifts, offering flexibility with a focus on providing compassionate, high-quality care to residents during weekend hours. The LPN Weekend Warrior collaborates with the care team to ensure that residents' needs are met, and their health and well-being are maintained. Key Responsibilities: Patient Care: Provide direct nursing care to residents, including administering medications, treatments, and monitoring vital signs during weekend shifts. Assist residents with activities of daily living (ADLs), including bathing, dressing, feeding, and mobility assistance. Respond to resident needs and emergencies promptly, ensuring timely and effective care. Monitor residents' conditions and report any changes to the supervising Registered Nurse (RN) or physician as needed. Medication Administration: Administer medications to residents as prescribed by physicians, ensuring accurate documentation and adherence to medication administration protocols. Monitor residents for any adverse reactions to medications and report concerns to the appropriate medical team members. Maintain accurate medication records, ensuring compliance with facility policies and healthcare regulations. Documentation and Compliance: Accurately document all patient care activities, including assessments, treatments, and medications, in the electronic health record (EHR) system. Ensure compliance with all federal, state, and facility regulations regarding nursing care and patient safety. Participate in shift handovers, ensuring that incoming staff are updated on resident conditions, treatments, and any critical information. Resident Advocacy and Support: Provide emotional support to residents, offering companionship and ensuring their comfort and dignity. Advocate for residents' needs and preferences, working with the care team to provide individualized care. Communicate effectively with residents' families, keeping them informed of any changes in health status or care plans. Team Collaboration: Collaborate with the nursing team, CNAs, and other healthcare professionals to provide comprehensive care to residents during weekend shifts. Participate in care planning meetings as needed, offering insights into residents’ weekend care and any changes in condition. Assist with weekend admissions, discharges, and transfers, ensuring a smooth and safe transition for residents. Qualifications Education: Graduate of an accredited LPN program. Licensure: Current and active Licensed Practical Nurse (LPN) license. Experience: Previous experience in a healthcare, long-term care, or assisted living setting is preferred but not required. Skills: Strong clinical and assessment skills, with the ability to provide high-quality nursing care. Excellent communication and interpersonal skills, with a compassionate approach to patient care. Ability to work independently and manage multiple priorities in a fast-paced environment.
