Position Summary:
The Children’s YCSU Case Manager provides goal-directed support, solution-focused interventions, and supervisory oversight to ensure delivery of high-quality community-based behavioral health and crisis stabilization services. Under the direction of the Director of Children’s Services, this position coordinates rehabilitation activities, oversees crisis stabilization programming, supervises assigned staff, and ensures clients receive individualized recovery-oriented services that promote stability and successful community integration.
This position will assist clients in achieving goals identified in their individualized treatment plans by reducing functional deficits and addressing interpersonal, environmental, and behavioral health barriers associated with mental illness and/or substance use disorders. Services may include direct rehabilitation, crisis intervention, advocacy, case coordination, transportation, and collaboration with families, community partners, hospitals, and other providers.
The position also provides day-to-day oversight of the Youth Crisis Stabilization Unit (YCSU) and 23-Hour Crisis Pods, coordinating admissions, discharges, staff assignments, quality improvement activities, licensing compliance, and communication with internal and external partners. Community-based services are provided in homes, schools, workplaces, and other community settings. Crisis response and on-call responsibilities are required.
Primary Job Duties and Responsibilities:
- Supervise daily operations of the Youth Crisis Stabilization Unit (YCSU), 23-Hour Crisis Pods, and assigned Community-Based Services staff.
- Provide leadership, training, scheduling, and oversight of assigned staff, including monitoring productivity, documentation, and required certifications.
- Assist in assessing client needs and developing individualized treatment plans.
- Implement rehabilitation interventions and assist clients in achieving treatment plan goals.
- Provide direct community-based rehabilitation and crisis stabilization services.
- Respond to psychiatric and behavioral health crises during business hours and participate in on-call rotation as assigned.
- Coordinate admissions, discharges, referrals, and transitions of care with Saveline, state hospitals, Psychiatric Residential Treatment Facilities (PRTFs), acute care hospitals, waiver staff, and other community partners.
- Complete discharge planning and ensure continuity of care through follow-up services and referral coordination.
- Assist clients in accessing community resources, including transportation when medically necessary.
- Advocate for clients to ensure appropriate services, benefits, housing, and community supports are obtained.
- Collaborate closely with clinical, medical, and psychiatric staff regarding medication services and treatment planning.
- Provide education and support to family members and caregivers.
- Coordinate transportation to hospitals, residential facilities, medical appointments, and other treatment-related services.
- Monitor program quality improvement initiatives and participate in performance improvement activities.
- Assist in monitoring departmental budgets and resource utilization.
- Maintain compliance with agency policies, CCBHC standards, licensing requirements, Medicaid regulations, and applicable state and federal guidelines.
- Ensure timely, accurate, and complete documentation in accordance with agency standards.
- Address client, family, and staff concern and assist in resolving grievances.
- Oversee housekeeping, safety, and environmental standards within crisis stabilization programs.
- Attend mandatory meetings, trainings, supervision, and professional development activities.
- Collaborate with other Community Mental Health Centers (CCBHCs), hospitals, law enforcement, schools, and community agencies to coordinate services and referrals.
- Perform other duties as assigned.
Credential and Experience Requirements:
- High school diploma or equivalent required.
- Bachelor's degree from an accredited college or university in psychology, social work, counseling, nursing, rehabilitation, or a closely related behavioral health field preferred.
- Relevant experience may substitute for education when appropriate.
- Minimum of two (2) years of direct experience in a Community Mental Health Center, crisis services program, or comparable public behavioral health agency required.
- Previous experience providing case management, community rehabilitation, crisis stabilization, or crisis intervention services preferred.
- State of Kansas certification required for applicable Community-Based Services.
- Annual certifications and requirements for agency training must be maintained.
- Valid Kansas driver's license and acceptable driving record required
Requirements
Performance Standards:
- Maintain an average of 100 direct service (billable) hours per month, averaged over a three-month period, when applicable.
- Meet productivity expectations for supervision, case conferences, utilization review, and administrative responsibilities.
- Demonstrate commitment to the Center's mission, vision, values, and recovery-oriented philosophy.
- Maintain required competencies, certifications, and continuing education through agency-approved training programs.
- Participate in quality improvement initiatives and program evaluation activities.
Required Skills/Abilities:
- Comprehensive knowledge of Community-Based Behavioral Health Services.
- Knowledge of crisis intervention, stabilization, rehabilitation, and recovery-oriented care.
- Knowledge of behavioral health treatment planning, case management principles, and evidence-based practices.
- Knowledge of the Kansas Mental Health System and available community resources.
- Knowledge of suicide prevention, violence risk assessment, and risk management.
- Ability to supervise, coach, and develop staff effectively.
- Ability to coordinate services across multiple agencies and treatment providers.
- Strong communication, organizational, problem-solving, and leadership skills.
- Ability to advocate for clients while maintaining professional boundaries.
- Ability to maintain confidentiality and comply with HIPAA requirements.
- Ability to work effectively with diverse populations in community settings.
Incumbent must:
- Be able to communicate effectively in spoken and written English language.
- Possess a valid driver's license.
- Operate usual office equipment.
- Ability to operate effectively within a wide variety of human service situations.
- Be able to lift and carry up to 25 pounds occasionally.
- Have the ability to move freely in the community and in a wide variety of terrain and circumstances. Special accommodation that can successfully assist an incumbent will be provided.
Additional Helpful Qualifications:
- Ability to communicate in languages other than English.
- Ability to communicate using American Sign Language.
- Knowledge that is culturally specific.
- Knowledge regarding self-help programs of recovery.
