Position OverviewÂ
The Case Manager ensures that Allies’ clients living with HIV/AIDS have access to necessary housing, food, medical, mental health, drug & alcohol, and legal services by engaging them in the service planning process, and providing education, counseling, and referrals which promotes optimal health and independent living. Additionally, this position will coordinate the permanent supportive housing program available through HUD (Housing and Urban Development). Allies provides medical and non-medical case management services to clients in Allegheny, Armstrong, Beaver, Butler, Cambria, Fayette, Greene, Indiana, Somerset, Washington, and Westmoreland counties. The position will work collaboratively with other Client Services staff, our Health Advocates, and a multitude of agencies.Â

Key ResponsibilitiesÂ
- Work with assigned clients to develop measurable goals and maintain an up-to-date service coordination planÂ
- Assess, monitor, evaluate, and record client progress according to measurable goals described in service coordination planÂ
- Assist clients in determining initial and ongoing eligibility for HUD assistanceÂ
- Communicate and enforce standards and contracts with clients and landlordsÂ
- Seek out and maintain relationships with landlordsÂ
- Maintain data in HMIS (Homeless Management Information System)Â
- Utilize techniques and principles of Harm Reduction, Motivational Interviewing, and Trauma Informed CareÂ
- Maintain certifications, documentation, and administrative paperwork in accordance with program policies and proceduresÂ
- Participate in annual HUD Monitoring processÂ
- Accompany clients to medical, housing, and social service-related appointments, and complete home visits and housing inspectionsÂ
- Link client to appropriate resources, in accordance with goals identified in service coordination plan, to provide access to services, and reduce barriers to careÂ
- Collaborate and coordinate with other professionals to assess and advocate for client’s medical, housing, and psychosocial needsÂ
- Advocate for clients to resolve crises and process emergency funding assistanceÂ
- Record all client interactions and services in electronic case management system Â
- Provide feedback to improve client servicesÂ
- Treat all clients, community members, and coworkers with dignity, respect, and unconditional positive regard – regardless of criminal history, substance use, sexual or gender identity, ethnic or racial background, or immigration statusÂ
- Participate in case conferences, HUD meetings, and Ryan White Collaboratives as appropriateÂ

Requirements
Knowledge Skills and AbilitiesÂ
The Housing Case Manager should have demonstrated experience in the following areas:Â
- Interviewing clients to gather information about their backgrounds, needs, or progress in order to develop and manage service coordination plansÂ
- Working with a multitude of agencies to coordinate referralsÂ
- Assisting clients from diverse backgrounds and circumstances in obtaining and maintaining housingÂ
- Basic knowledge and understanding of AIDS/HIVÂ
- Basic knowledge of local resources, referrals, and service systemsÂ
- HIPAA and Act 148 standardsÂ
- Customer service, conflict resolutionÂ
- Harm ReductionÂ
- Working with Electronic Health Records SystemsÂ
- Strong communication and writing skillsÂ
- Balancing and prioritizing patient and administrative responsibilitiesÂ
A Bachelor’s Degree in social work or similar area is required; an MSW is preferred.  Â
Previous experience working with HUD programming and HMIS are preferred. Â
All Case Managers must have a current driver’s license, auto insurance, and access to a reliable personal vehicle.  The Case Manager should expect to work minimal non-traditional hours and will be called upon to engage in physical activity related to working in an office setting including the capacity to lift 25 lbs. and stand or sit for extended periods. Current PA child abuse, PA criminal history, and Federal criminal history clearances are required within the first 30 days of employment.Â
Salary for this position begins at $45k-$52k commensurate with relevant education, experience, and licensure. We comply with EEOC.Â
