Job Description:

Vitalant is seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join the Hemophilia Center of Western Pennsylvania. Your Impact

As a Licensed Clinical Social Worker, you will play a vital role in improving the quality of life for individuals with inherited bleeding and clotting disorders. Working as part of a multidisciplinary care team, you'll provide psychosocial assessments, counseling, care coordination and advocacy to patients and families across all stages of life. Your expertise will help patients navigate the emotional, financial and social challenges associated with chronic medical conditions while connecting them with the resources they need to achieve the best possible outcomes.

  • Monday – Friday 8:00 a.m. – 4:30 p.m.

Starting Pay: $65k - $78k Based on Years of Experience

As a Licensed Clinical Social Worker, You Will:

  • Provide psychosocial assessments and ongoing counseling to patients and families affected by inherited bleeding and clotting disorders.
  • Develop individualized care plans and collaborate with physicians, nurses, genetic counselors and other healthcare professionals to support comprehensive patient care.
  • Connect patients and families with community resources, financial assistance programs, insurance resources and other supportive services.
  • Assist patients in navigating complex healthcare systems and overcoming barriers to treatment and care.
  • Provide crisis intervention and emotional support during periods of medical, financial or personal stress.
  • Educate patients and families on coping strategies, adjustment to chronic illness and available support resources.
  • Advocate for patients and families to promote access to quality healthcare and community services.
  • Maintain accurate and timely documentation in accordance with organizational policies and regulatory requirements.
  • Participate in multidisciplinary clinics, quality improvement initiatives and community outreach activities.

What We're Looking For

Minimum Qualifications

Education

  • Master's Degree in Social Work (MSW) from an accredited program required.

Licensure

  • Current Licensed Clinical Social Worker (LCSW) or Licensed Social WOrker (LSW) licensure in the Commonwealth of Pennsylvania required.

Experience

  • 3 years of relevant work experience is required. Can use clinical rotations. Clinical social work experience preferred.
  • Experience working with individuals living with chronic medical conditions, rare diseases or complex healthcare needs preferred.
  • Experience in a hospital, outpatient specialty clinic, hematology, oncology or pediatric setting is highly desirable.

Knowledge, Skills & Abilities

  • Strong clinical assessment, counseling and crisis intervention skills.
  • Knowledge of psychosocial issues affecting individuals and families living with chronic medical conditions.
  • Ability to build trusting relationships with patients, families and interdisciplinary care teams.
  • Excellent communication, organizational and documentation skills.
  • Strong understanding of community resources, insurance programs and patient advocacy.
  • Ability to maintain confidentiality while exercising sound clinical judgment.
  • Compassionate, patient-centered approach with a commitment to improving the lives of those within the bleeding disorders community.
