The Outpatient Nurse Coordinator at ELFHCC is responsible for overseeing and coordinating nursing services to ensure high-quality patient care. The Outpatient Nurse Coordinator functions as part of the care management team, performing a variety of clinical and administrative duties related to patient care. Duties may include quality monitoring, patient assessments, direct patient care, and patient/caregiver/staff education. This role exercises independent judgment and engages in continued communication with the patient, family, providers and community resources to assist the patient in attaining a measure of outpatient health. Qualified bilingual candidates are strongly encouraged to apply.


QUALIFICATIONS

  • Bachelor of Science in Nursing (BSN) or equivalent. Some roles may require a Master of Science in Nursing (MSN).
  • Minimum of 3 years of nursing experience, with a focus on coordination or management roles.
  • Strong knowledge of nursing principles, healthcare regulations, and ELFHCC policies is essential.
  • Assist physicians with all aspects of patient care including assessment, evaluation, and education.
  • Formulates goals and a plan of care for patients that involves the patient as a partner and considers the individual needs of the patient in a holistic manner.
  • Experience with diverse patient populations and understanding of community health issues.
