Job Description : 

GENERAL OVERVIEW:

The Mental Health Professional will provide clinical treatment services to the targeted populations. This is an outpatient treatment program designed to service patients with a mental health diagnosis who meet dimensions for outpatient and/or partial hospitalization treatment criteria.

ESSENTIAL RESPONSIBILITIES:

  • Adheres to all regulatory requirements while developing and facilitating assessments and/or therapeutic interventions (individual, group, family), treatment plans, and encounter forms for assigned patients and/or providers. Ensures an effective continuum of services, guided by clinical decisions, while strictly adhering to all institute and regulatory guidelines for documentation, treatment, and case management, and continuously adapting treatment concepts for optimal service delivery (70%)

  • Discusses need for additional services with the clinical team to determine appropriateness of recommendations. Refers patients to other levels of care, community-based self-help programs for recovery or other community social or human services as needed. (10%)

  • Acts as a liaison, both internally and externally, to professional services to ensure patient care requirements are met. Participates in external and internal meetings as assigned. (10%)

  • Participates in the quality assurance and performance improvement activities of the program. (10%)

QUALIFICATIONS:

Minimum

  • Master’s degree in Psychology, Social Work, Counseling, Education (School Counseling), or related field

  • CPR – American Heart Association

  • Act 34 Criminal Background Clearance Certificate

  • Act 33 Child Abuse Clearance Certificate

  • Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
