We are seeking a skilled and compassionate Physical Therapist to join a Skilled Nursing Facility in Placerville, California. This role involves working with a geriatric and post-acute population, helping patients recover from surgery, illness, or injury while improving mobility and overall function. You will be responsible for evaluating patients, developing individualized treatment plans, and guiding them toward greater independence and quality of life. Job Responsibilities:
• Evaluate, diagnose, and treat patients with physical impairments in a Skilled Nursing Facility setting
• Develop and implement individualized treatment plans based on patient needs and clinical findings
• Supervise and collaborate with Physical Therapist Assistants to ensure effective care delivery
• Monitor patient progress and modify treatment plans as needed
• Educate patients and caregivers on mobility, safety, and home exercise programs
• Maintain accurate and timely documentation in compliance with facility and regulatory standards


Requirements

• Active state license as a Physical Therapist in California
• Current BLS/CPR certification
• Minimum of 1 year of recent PT experience preferred, ideally in a SNF or geriatric setting
• Strong clinical reasoning and leadership skills
• Ability to work collaboratively within an interdisciplinary team
