To actively engage with individuals to achieve vocational, educational, and ADL goals. The Community Connection Specialist (CCS) primary role is to provide support necessary for people to achieve their desired lifestyle and increased levels of independence, productivity, and social/physical integration in their community. Reports to the Program Director. Adhere to all policies, procedures, protocols, and rules set forth by Arrow Services Indiana. Implement Arrow Services Indiana’s mission and vision statement and the core values into all aspects of the position.
RESPONSIBILITIES
- Complete 90-day orientation.
- Performing personal care tasks, including assistance with basic personal hygiene and grooming, feeding, and ambulation, medical monitoring, and health care-related tasks. Fostering positive relationships between individuals served and their housemates
- Ensuring individual safety and maintaining a safe environment
- Assistingwith toileting, including bedpans, urinals, and commode chairs
- Reminding/assisting individual with self-administration of medications (with proper training)
- Encouraging self-help activities
- Reporting changes in individual's condition or family situation to the office
- Documenting services provided
- Performing home management functions such as light housekeeping, laundry, bed making, and cleaning
- Planning meals, shopping for groceries, preparing and serving food/meals, feeding, and clean-up
- Accompanying individual(s) to scheduled appointments
- Transporting individual(s) or running errands for individuals
- Demonstrate ability to follow oral and written instructions
- Implement the individual’s support plan to reach their goals.
- Teachand/assist with daily living skills as necessary.
- Effectively identify and respond to challenging behaviors outlined in the individual’s Behavioral Support Plan.Â
- Collaborate with Program Director on community activities for the individual’s interest and needs
- Participate in annual trainings
- Participate in annual surveys
- Participate in annual assessments for the individuals served.
POSITION REQUIREMENTS:
MINIMUM
- Must be at least eighteen (18) years of age.
- Must be able to lift 25+ pounds
- Ability to handle physical aspects of a job, including bending, stooping, lifting, pulling, reaching, and walking for periods of time.
- Demonstrate ability to provide services according to the individual ISP
- Demonstrate willingness to accept supervision
- Demonstrate an interest in and empathy for individuals.
- A negative TB screening dated prior to the employee providing services for all employees/agents, including administrative and clerical staff.
- Demonstrate the ability to communicate adequately to:
- Complete required forms and reports of visits; and
- Follow oral or written instructionsÂ
- Must possess a valid driver’s licenseÂ
- Must meet all agency requirements for pre-employment as required by Arrow and/or State regulations.
- Ability to communicate (verbally and in writing) with all levels of personnel, internal and external to the company 
- Available to fulfillafull time (32 – 40 Hour) work week, or a part-time (1-31 Hour) work week.
