Role and ResponsibilitiesÂ
The Clinical Field Staff Supervisor (CFSS) is a RN with certified home health care experience. The CFSS plays a  integral role of the patient care team and continually monitors the services provided. This role has shared responsibility for the overall provision and coordination of home health care services provided to HCR's patients, according to acceptable standards of practice. The CFSS is a resource person with extensive knowledge of the regulatory guidelines governing certified home health care agencies and provides daily support to field clinicians, enabling them to provide quality care in the home to HCR patients. The CFSS is a liaison between referral resources, patients, caregivers, and employees. Â
Essential FunctionsÂ
- Receive calls from field staff and physicians regarding client care issues/orders. Â Enter orders as appropriate and update the plan of care, medication profile and schedule. Enter information regarding all telephone calls into the coordination notes section of the computer software. Notify staff, as appropriate, to ensure proper coordination of care.Â
- Review and approve all interim orders before they are sent to physician. Notify Clinical Manager when unable to approve the order.Â
- Provide support to the Intake staff and process referral requests in HCHB as needed. In branch offices, processing referrals is a shared responsibility of the PSC and CFSS roles.Â
- Assign Licensed Professional (LP) to conduct the evaluation visit as the task appears on the action screenÂ
- Participate in case conferences, as needed, and process recert/discharge decisions based on clinical documentation and input from case manager, based on case orders for tracking purposes and route to Medical Records to be sent to the physician for signature. Ensure any corrections are made by the LP who wrote the order prior to entering the order. Update the client’s medication profile and schedule as applicable; via the order. Ensure all orders that address frequency have appropriate calendar modification completed.Â
- Follow-up on orders from the Order Tracking Report when the MRS is unable to retrieve the unsigned order.Â
- Review and follow-up on entitlement verification issues.Â
- Verify/Ensure there are orders for requested medical supplies, enter supply orders and requisition supplies as needed.Â
- Enter non-admit information into HCHB, with details in coordination notes if no visit was made.Â
- Review missed visit reports, ensure MD notification has occurred, and if not, notify MD.Â
- Other duties as assigned.Â

Education RequirementsÂ
- Associate’s Degree in Nursing required. Bachelor’s Degree in Nursing preferred.Â
Qualifications and RequirementsÂ
- Current, valid RN license, issued by NYS Department of Education required.Â
- Two (2) years’ experience in the health care industry with a minimum of 1 year experience in certified home health care. Supervisory experience is preferred.Â
- Understands issues related to the delivery of certifed home health care services and is able to problem-solve effectively.Â
- Possesses knowledge of the Medicare guidelines governing certified home health care agencies.Â
- Is organized, detail-oriented and possesses strong interpersonal, organizational, time management, as well as written/verbal communication skills.Â
- Is able to work independently and complete paperwork accurately and in a timely fashion.Â
- Possesses strong basic computer skills and has the ability to use agency’s EMR system.
- Must comply with privacy and HIPAA laws.Â
- Valid driver’s license and automobile with current insurance coverage and in good working condition, available to use for work.Â
- Willingness to submit to physical examination, criminal background and drug testing.Â
Work EnvironmentÂ
The CFSS is primarily in an office setting and may be exposed to outdoor conditions.Â
The working conditions are classified as light work:Â
- Light work - Exerting up to 20 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Light work involves sitting most of the time.Â
