Job Description:

Duties/Responsibilities:

  • The role of the pharmacy technician in medication reconciliation is to accurately and efficiently:
    a.    Collect patient’s medication information from home and current pharmacy.
    b.    Create and log patient’s complete home medication list.
    c.    Clarify for accuracy and completeness.
    d.    Document patient’s complete home medication list, retail pharmacy and vaccination history if available.
  • Expected to perform most job duties independently, in accordance with established departmental and hospital policies and procedures.
  • Must demonstrate special skills required to efficiently interact with patients and families from different backgrounds and age categories.
  • Assists in the preparation of irrigation, intravenous admixture solutions, and other, using aseptic technique.
  • Fills unit dose, Automated Dispensing Machines (ADM), stock requisitions and maintain inventory levels in OR, Endo & OB.
  • Delivers unit dose, ADM refills, intravenous solution and necessary pharmaceutical supplies to the nursing units.
  • Fills medication labels, subject to final check by a pharmacist.
  • Prepares pre-packaged medications in commonly used quantities.
  • Assists with inventory control of supplies and medications. Receives and shelves supplies and medications. Performs cycle counts and monthly review of stock for expired medications.
  • Cleans pharmacy equipment, utensils and work areas.

Education/Experience Required:

  • Must be 18 years of age or older.
  • Ability to read, write and understand English at the 12th Grade level, follow orders, and written and illustrated instruction.  Ability to perform arithmetical computations (add, subtract, etc.).
  • Must have passed Board of Pharmacy Technician Registration exam; all requirements of Pharmacy Technician Trainee
  • Must know basic IV admixture processes as well as sterile procedures.
  • Must have good math, computer and communication skills.
  • Must have clear understanding of medication names (brand and generic), common dosing and sig codes, and medical terminology.
  • Current PTCB certification or completion of 2 years of pharmacy school.
  • Must score 85% or greater on Medication Reconciliation Technician Test

Registration/Certification:

  • Must be registered by the Massachusetts Board of Pharmacy as a Technician or as Pharmacy Intern.
  • Must be certified by the PTCB (Pharmacy Technician Certification Board) or have completed 2 years of pharmacy school.
  • Must maintain license from Board of Pharmacy and continuing education requirements for PTCB.

Physical/Environmental Requirements

This job requires frequent bending, carrying, pulling, pushing, reaching, standing, stooping, walking and lifting over 10 lbs. There will be occasional need for climbing stairs, lifting in excess of 50 – 100 lbs and sitting. Constant use of hearing, sight and speech. Frequent use of smell and touch. Frequent keyboard and telephone use. Constant concentration and problem solving.

 

 

Pay Range:

$21.27 - $38.03

This position is covered by a collective bargaining agreement. The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time.  The pay range for this position is established and governed by the terms of the applicable collective bargaining agreement. Employees hired into this position will receive compensation consistent with the terms of the applicable collective bargaining agreement.
