Job Description:

1. Clinical Responsibility & Triage

• Adheres to the established Responsibility and Priority Hierarchy for treatment and triage of surgical patients (see Supplement 1).

• Collaborates closely with the Attending Surgeon and surgical team to identify, assign, and assess patient care needs.

• Provides assistance with:

  • Established inpatient consults

  • Rounding

  • Discharge planning

  • Patient management
    when patient volume and acuity allow.

• When workload or acuity exceeds capacity, responsibilities defer back to the Responsibility and Priority Hierarchy and Attending Surgeon.

• Maintains ultimate responsibility for identifying and triaging assigned patients.

2. Invasive & Surgical Procedures

• Performs major invasive procedures under specific written protocols developed collaboratively by:

  • Chief Medical Officer

  • Medical Director of Surgery

  • Chief of Surgery

  • Administrative Program Leadership

  • PA/MD (see Supplement 2)

• Procedural privileges are granted based on:

  • Prior documented competency, or

  • Completion of the Invasive Procedural Competency Pathway

• Each competency pathway requires formal assessment and acknowledgment by supervising clinical leadership.

3. Surgical Consultations

• Conducts surgical consultations in accordance with written protocols developed by:

  • Chief Medical Officer

  • Medical Director of Surgery

  • Administrative Program Leadership

  • PA/MD

• Refer to Supplement 3 for consultation guidelines.

4. Qualifications & Eligibility

The Surgical House Officer may be a Massachusetts-licensed:

• Physician Assistant (PA)

• Nurse Practitioner (NP)

• Physician

Must meet minimum organizational procedural and clinical competency requirements, including but not limited to:

Required Experience

• Co-Surgeon and First Assistant proficiency across all surgical subspecialties

• Inpatient consultation, treatment, and triage experience

• Surgical Critical Care experience

Required Procedural Competencies

• Central venous catheter placement (Internal Jugular, Subclavian, Femoral) with ultrasound guidance

• Hemodialysis catheter placement (Internal Jugular, Subclavian, Femoral) with ultrasound guidance

• Thoracostomy tube placement:

  • Traditional Argyle

  • Seldinger technique (Rocket/Wayne)

Preferred / Additional Procedural Competencies

• Thoracentesis

• Paracentesis

• Emergency surgical airway management

• Advanced Trauma Life Support (ATLS)

5. Duties & Responsibilities

• Perform comprehensive and focused histories and physical examinations at the request of the Attending Surgeon

• Diagnose and manage surgical illnesses and injuries

• Order, interpret, and evaluate laboratory and radiographic studies

• Educate and counsel patients regarding surgical procedures and post-operative care

• Perform minor surgical procedures outside the operating room (including use of MSTA when appropriate)

• Serve as First or Second Assistant for all surgical subspecialties

• Provide emergency surgical care

• Facilitate referrals to appropriate physicians or agencies

• Prescribe medications in compliance with state law and hospital policy

• Conduct daily hospital rounds as required

• Write orders and progress notes as needed

• Communicate patient status updates to the Attending Surgeon

• Develop and implement treatment plans for hospitalized patients

• Manage discharge planning and dictate discharge summaries

• Ensure safe, effective patient handoffs consistent with standards of care

Pay Range:

The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.  Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
