Job Purpose
The UBHCC Nurse is responsible for supporting the multidisciplinary crisis team by delivering coordinated, high-quality care to clients at the Urgent Behavioral Health Crisis Center.Â
QualificationsÂ
- Must be a licensed LPN.
- Experience in psychiatric or behavioral health care and healthcare technology is preferred.
- Ability to conduct basic nursing assessments.Â
    Core Job Duties
- Obtain and record vital signs.
- Perform blood and urine collection as requested by prescribers.
- Reconcile home medications.
- Complete triage assessments.
- Based on triage assessment outcomes, determine appropriate care pathways and facilitate timely engagement with the next provider to ensure efficient service delivery.
- Monitor clients’ physical and behavioral health status during their visit.
- Communicate relevant information to the clinical team.
- Conduct violence risk screening using the agency screening tool.
- Complete nursing assessments.
- Review information from PDMP (Prescription Drug Monitoring Program).
- Provide medication education.
- Facilitate telehealth connections for Nurse Practitioner evaluations.
- Call in prescriptions and refill medications as directed by prescribers.
- Complete documentation in a timely manner.
- Communicate information orally in an effective and accurate manner.
- Assure program operates with the constraints of the agency’s certification (CARF, DMH) standards and strategic plan related to clinical care.Â
- Abide by program and agency policies and procedures.
- Other Duties: Participate in special projects and agency initiatives as assigned.
