Job Summary
We are seeking a Registered Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest.Â
Service Area: Port St. Lucie to Sebastian, FL

ResponsibilitiesÂ
- Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plansÂ
- Oversee and support in-home caregiver teams
- Ensure compliance with care plans and agency standards
- Document any changes in patient conditions and report on changesÂ
- Maintain accurate records of patient care and discuss observations with the supervisor Â
Qualifications
- Currently licensed Registered Nurse (RN) in state of employmentÂ
- Previous experience as a Registered Nurse is preferred
- Excellent interpersonal and client care skills
- Valid driver’s licenseÂ
- First aid/CPR certification requiredÂ
- English required; Spanish and Creole speakers a plus
- Reliable transportation
- Ability to pass a Level 2 Background Check and I-9 verification
