The Hospice Access Field and Triage RN will complete medical requirements while coordinating a whole-person plan of care. Care is according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act. In Field role, the RN will visit patients in their homes (private or facility) to present the hospice program and conduct eligibility assessments, coordinating care with Hospice MD, family and facility caregivers, and interdisciplinary team (IDT). Field RNs will also make urgent and routine home visits to patients already on hospice service. In Triage role, RNs will take patient calls from Halsey Building in NE Portland.Â

Required Qualifications:
- Education sufficient for licensure
- Upon hire: Oregon Registered Nurse LicenseÂ
- Within 6 months (180 days) of hire: Washington Registered Nurse LicenseÂ
- Upon hire: National Provider BLS - American Heart AssociationÂ
- Upon hire: In compliance with the Providence Health and Services-OR, criminal background check is required for this position, based on assigned duties.Â
- 2 years Acute care and/or hospice, home health, or long-term care RN experience
Preferred Qualifications:
- Bachelor's Degree Nursing
- Call Center experience.
