This position will be traveling to see patients in the Portland Metro area (primarily East Portland).
This role works under the direction of a registered nurse to provide quality nursing care to patients in their homes. Assists with collection of assessment data and provides input to the planning of care. Implements plan of care within the scope of licensure. Performs all duties in a manner which promotes team concept and reflects the Providence mission and philosophy.

Required Qualifications:
- Graduate of an accredited LPN/LVN school of nursing.
- National Provider BLS - American Heart Association within 30 days of hire.
- Oregon Licensed Practical Nurse upon hire.
- Driving is a necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request.
- 1 year - Experience in nursing; long term care, acute care, ambulatory care or home health and hospice care.
Preferred Qualifications:
- Experience in home health, hospice or public health.

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
