Patient Navigator at Providence Portland Medical Center- Full-Time, Day ScheduleÂ

The Patient Navigator provides person-centered service planning which links vulnerable individuals to the services and resources they need to strengthen their health, stability, and connectedness to family and community. The Patient Navigator will assist patients and their families in managing through the vast array of services and programs offered by the hospital and the community to meet their health and wellness needs. The Patient Navigator will provide personalized, high touch care, advocating the integration of whole person health of body, mind, and spirit. The Patient Navigator will tailor services to the unique wellness needs of the patients served. The Patient Navigator will participate in health screenings, provide patient education, and offer appropriate referral information. The Patient Navigator will be responsible for cultivating relationships with patients and their families, physician partners, members of the healthcare team, and community partners. The Patient Navigator will be responsible for collection of data related to contacts and referrals as outlined by the department manager. The Patient Navigator will consistently demonstrate behaviors that model the PSJH core values and the hospital's mission statement.

Required Qualifications:
- H.S. Diploma or GED
- Within 1 year of hire Oncology Patient Navigator Certification.
- 5 years' experience in a health care setting.
Preferred Qualifications:
- Associate's Degree in a health care related field.
- Experience with planning and conducting in-servicing education and training.
