Summary of Job Responsibilities: (Not all inclusive)
       I.        Interdisciplinary care teams
a.      The Pharmacist may participate in interdisciplinary care rounds or other committee meetings such as medication safety, antimicrobial stewardship, Pharmacy and Therapeutics, Stroke, Sepsis, etc.
b.      Works collaboratively with the interdisciplinary team to maximize individual patient outcomes.
     II.           Performs review of patient’s home medications at admission
a.      Compares medication history obtained by pharmacy assistant against inpatient orders to ensure accuracy.
b.      Works with physicians on discrepancies found between inpatient orders and medication reconciliation and documents outcomes.
   III.           Medication therapy monitoring
a.      Ensures drug regimens are appropriate and follows hospital established policies/procedures/protocols or evidence-based guidelines in the absence of hospital policies/procedures/protocols. This includes but is not limited to compliance with antimicrobial use guidelines, IV to PO interchanges, renal dosing, and serum drug concentration protocols.
   IV.           Review of medication orders
a.      The Pharmacist evaluates medication therapy, identifying medication related problems. All medication orders are reviewed for the appropriate dose, duration, frequency, dosage form and indication.
b.      Monitors therapy for potential or actual drug interactions and contraindications.
c.       Monitors selection of safe, appropriate, cost-effective medication therapies according to established formulary, policies and procedures.
     V.           Provides drug information to health care providers.
a.      Answers general and patient specific drug information questions.
   VI.           Sterile compounding
a.      Compound sterile intravenous compounds while adhering to 797 regulations.
Other duties as assigned. Pharmacists supervise the work of technicians, may be involved in policy and procedure upkeep and development, controlled substance diversion monitoring, ambulance restocks, order set review and development. May be asked to participate in education programs as needed.

Qualifications:
o  Doctor of Pharmacy (PharmD) degree from an accredited pharmacy school.
·        Licensure:
o  Valid state license to practice as a pharmacist in New York.
·        Experience:
o Preferred, not required): 1 to 2 years hospital experience.
